As part of our overall Safety Management System (SMS), California Pacific Airlines has established a system for our Flight Attendants, Dispatchers, Ground Operations, Pilots, Maintenance Personnel, and Administrative Employees to voluntarily identify and report safety and operational concerns. The collected information is reviewed and analyzed to facilitate early detection and improved awareness of operational deficiencies and adverse trends. The information specified in employee reports is used to identify the root causes and determine appropriate remedial actions, which are then monitored for effectiveness. This process promotes collaboration between employee work groups and management for the early identification of hazards and to maintain a proactive approach regarding safety concerns and corrective action recommendations
California Pacific Airlines, in cooperation with the FAA, has established an Aviation Safety Action Program (ASAP) for all operational employee groups. The California Pacific Airlines ASAP is a voluntary, self-reporting program designed to identify and reduce possible safety concerns. ASAP uses employee input to identify significant safety concerns and issues; operational deficiencies; non-compliance with regulations; deviations from company policies and procedures; and unusual events. In partnership with the employee groups and the FAA, each report is investigated and corrective actions determined based on a non-disciplinary approach to flight safety.
California Pacific Airlines has developed a system for the submission of incident and event reports by our Flight Attendants, Dispatchers, Ground Operations, Pilots, Maintenance Personnel, and Administrative Employees to identify safety and operational deficiencies within their respective departments. These reports will be investigated and analyzed to identify hazards; determine appropriate safety and/or operational improvements; monitor the effectiveness of corrective actions, and proactively promote employee awareness of potential problems.
Employee work-related injury/illness incidents must be reported by submitting a WBAT Injury/Illness Form within forty-eight (48) hours of the incident. Initial verbal or e-mail notifications are acceptable, but must be followed up with a written injury report as soon as possible. If a crewmember becomes incapacitated at any time during the trip, contact the OCC immediately. They will notify the operations managers.
Employee safety concerns may be anonymously submitted through the Safety Hotline at (404) 410-7612 ext. 400. Valid Safety concerns will be acted on accordingly.
In an effort to prevent a Safety Hazard from escalating to an incident, California Pacific Airlines has included a Safety Improvement Report for each Employee Group. If an event or situation is witnessed that could be a hazard, filing a Safety Improvement Report will allow key individuals within the Safety Department to evaluate the situation. With the help of Department Managers, corrective action can be taken to mitigate unnecessarily high-risk events and make the working environment safer. All information submitted through the Safety Improvement Reporting system will remain confidential.
